After you apply

After you submit your online application, we will:

• Review your application and contact you for additional or clarifying information
• Contact you to initiate a participating agreement
• Ask you to:
  • Review the participating agreement
  • Sign and return it to us

Credentialing may be required before an agreement becomes effective. We may ask you to submit additional information to complete credentialing. Once credentialing is complete, you will:

• Get a copy of the contract, which will be signed by a Humana representative
• Learn your effective date with Humana

When applying to join our network, you also must complete the Ohio Department of Medicaid’s  Medicaid addendum .

Credentialing process on or after July 1, 2022

The Ohio Department of Medicaid (ODM) is responsible for credentialing all Medicaid Managed Care providers. The credentialing and recredentialing processes are paired with enrollment and revalidation, respectively, in the Provider Network Management system. This process adheres to National Committee for Quality Assurance (NCQA) and Centers for Medicare & Medicaid Services (CMS) federal guidelines for both processes and the types of providers who are subject to the credentialing process.

Please note, you are not able to render services to Medicaid members until you are fully screened, enrolled, and credentialed (if required) by Ohio Medicaid. For a complete list of provider types that require credentialing, please refer to  Ohio Administrative Code (OAC) rule 5160-1-42 .

Join our dental network

DentaQuest administers the Humana Healthy Horizons in Ohio dental plan. To join our dental network:

• First,  create an account in the Ohio Medicaid credentialing Provider Network Management module , and then
• Register with DentaQuest for Humana:
  • By using the  DentaQuest online provider portal , or
  • By calling the Humana DentaQuest provider phone line at 833-615-0432, Monday – Friday, 7 a.m. – 8 p.m., Eastern time

Existing DentaQuest providers who want to join the Humana Healthy Horizons in Ohio dental network can contact their DentaQuest representative for more information.
