Member out-of-service-area notification

Please notify Humana Healthy Horizons when you become aware one of your patients with Humana Healthy Horizons in Florida coverage moves out of your service area. Please complete this notification form  and submit by fax or email.

Notification of deceased member

Humana Healthy Horizons in Florida network-contracted healthcare providers should use the Notification of Deceased Member Form  to notify us when a member dies.

Sample patient service letters

Patients miss valuable medical services and tests for many reasons. Humana has developed physician tools to help patients keep their Managed Medical Assistance (MMA) coverage current. The adult sample services letter provides a checklist of recommended health services and tests for adult MMA members that includes preventive screenings, medications and diabetes care. 

The pediatric sample services letter provides a checklist of the recommended health services and tests for pediatric MMA members, including preventive screenings and medications.

Healthcare providers can: 

  • Download and customize the letter(s) for their practice letterheads, communication styles and patients’ needs
  • Distribute the letter(s) to their MMA-covered adult and pediatric patients regardless of their patients’ health benefit plan membership

Virtual care

Humana is looking for physicians who currently provide telemedicine services or would like to learn more about this convenient alternative for Medicaid patients. Maternal Opioid Recovery Effort (MORE)

According to the FPQC, overdoses are the leading cause of pregnancy-associated death in Florida. The purpose of MORE is to improve identification of—and clinical care and coordinated treatment/support for—pregnant women with opioid use disorder and their infants.
