Sample patient service letters
Patients miss valuable medical services and tests for many reasons. Humana has developed physician tools to help patients keep their Managed Medical Assistance (MMA) coverage current. The adult sample services letter provides a checklist of recommended health services and tests for adult MMA members that includes preventive screenings, medications and diabetes care.
The pediatric sample services letter provides a checklist of the recommended health services and tests for pediatric MMA members, including preventive screenings and medications.
Healthcare providers can:
- Download and customize the letter(s) for their practice letterheads, communication styles and patients’ needs
- Distribute the letter(s) to their MMA-covered adult and pediatric patients regardless of their patients’ health benefit plan membership
