Coverage of at-home COVID-19 test kits

We cover costs associated with at-home COVID-19 test kits that you order for one of our members in your panel. A prescription is required. Coverage is available for at-home test kits that have emergency use authorization or U.S. Food and Drug Administration approval. Eligible at-home test kits are kits available in package sizes of one or two at-home test kits.

Rewards available for our eligible members who receive a COVID-19 vaccine

All members 6 months old and older enrolled in Go365 for Humana Healthy Horizons® can earn $20 in rewards after submitting proof of receiving a COVID-19 vaccine.

To be eligible for this reward, our members must:

  • Register an account through  Go365 for Humana Healthy Horizons®
  • Be completely vaccinated against COVID-19 (number of doses depends on person’s age)
  • Log in to their  Go365 for Humana Healthy Horizons account
  • Upload an image of their completed vaccination card
