Humana Medicare Advantage (MA) supplemental dental benefits are provided through the HumanaDental Medicare Network for plans purchased outside of Florida, and the Florida GoldPlus Dental Network for plans purchased in Florida.

For Dental Offices

For Medicare eligibility/benefits and claims inquiries, call 800-833-2223, Monday – Friday, 8 a.m. to 8 p.m., Eastern time.

Humana’s MA member ID card indicates the dental plan (DENxxx) on the back of the card. All of Humana’s DENxxx plans use a PPO dental network. Benefits may be in-network only or in-and out-of-network. (The plan name, including HMO or PPO, on the front of the ID card only refers to the medical benefits.)

We prepared a handbook that provides the information you need to administer these benefits. The handbook also includes useful information for dental providers, such as:

  • Humana Medicare Advantage dental (DEN) benefits, showing American Dental Association (ADA) codes and frequencies
  • How to contact provider customer service
  • How to access the secure dental provider site
  • How to submit claims
  • Payer codes
  • How to review the dental provider directory and update information if necessary
  • Dental network partners
