Join Humana’s behavioral health network

  1. Complete and submit our join the network form .
  2. A Humana representative will send you contracting documents to complete.
  3. Update your CAQH profile. Please note, credentialing can take 60−90 days.
  4. For an update on the status of your application, log in to Availity to see if you are active or fill out our inquiry form .
  5. Once contracted, you’ll receive a provider welcome packet.
