Enrollment with the Department of Medical Assistance Services (DMAS) via the Provider Services Solution (PRSS) portal

Participation in our network requires enrollment through the DMAS PRSS enrollment wizard. 

  • Select Humana Healthy Horizons® in Virginia under managed care organization.

Once notified, Humana Healthy Horizons will assist in completing the enrollment process.

Join our network

To join our network, visit our Join our Network page, and select the link under “Apply to Humana’s medical network.”

Steps to joining our network:

  1. To initiate the network enrollment process, please complete the online form, which includes a list of required documents.  
  2. After submission, a confirmation email and tracking number will be provided. A contracting representative will follow up to assist with the next steps.  
  3. The application will be reviewed for completeness. If anything is missing, the provider will be notified within 30 days.  
  4. Once complete, a credentialing decision will be made within 60 days, and a decision letter will be mailed confirming the process is complete. 

Please note: Humana Healthy Horizons will process claims for covered services rendered to a Humana Healthy Horizons member during the period of a pending application no later than 40 days after the provider applicant is credentialed and contracted.
