Once we receive your inquiry, a member of our contracting team will contact you to discuss the process of becoming a Humana Healthy Horizons network provider.

To participate with Humana Healthy Horizons, all providers must enroll through the Department of Medical Assistance Services (DMAS) Provider Services Solution (PRSS) enrollment wizard. You will be asked which managed care network you would like to join, so be sure to select <Humana Healthy Horizons in Virginia>. After DMAS notifies Humana Healthy Horizons of your request, we will send an enrollment package to you.
