Join our physical and behavioral health network

Steps for a new provider to join our physical health or behavioral health network:

  1. To initiate the network enrollment process, please complete the request to join online form which includes a list of required documents. 
  2. After submission, a confirmation email and tracking number will be provided. A contracting representative will follow up to assist with the next steps.

Participating providers seeking to add providers to an existing group should submit a roster and W-9 form via email to:

Join our pharmacy network

Please email our Pharmacy Contracting team at PharmacyContractingRequests@humana.com and include “Join the network” in the subject line.

Join our LTSS network

Humana is partnering with the Detroit Area Agency on Aging (DAAA) for administration of our Medicaid waiver network services in Michigan Region 10 (Macomb County) and Region 12 (Wayne County). Home- and community-based services (HCBS) providers should initiate requests to join the network for Humana HMO D-SNP members directly with DAAA by emailing contractmgt@daaa1a.org

HCBS providers serving all other Michigan counties should contact the Humana LTSS Contracting team by emailing their network interest to LTSSContracting@humana.com

To join the TruHearing network, contact TruHearing Network Management at 866-581-9462 (TTY: 711).

The TruHearing team will advise you on credentialing, provider reimbursement, etc.

The process can take as little as 2 weeks depending on your current credentialing status and how quickly you provide the requested responses.
